Each season your consent and other forms must be turned in before participation is allowed. Some of the forms are pre-filled in for you during the registration process other forms must be printed from this site and turned into your child’s coach at the beginning of the season.

Diocesan Sports Parental Consent Form – Required for all Diocesan sports and must be submitted via the DRA sports portal.
Club Sports Parental Consent Form – Required for all Club sports, this is pre-filled and submitted for you on the registration web site.
Medical Consent Form – Required for all sports, this is pre-filled in for you on the registration web site. Print and sign this form and turn it into your child’s coach.
Ohio Dept. of Health Concussion Information Sheet – Required for all sports. Print and sign this form and turn it into your child’s coach.
